Beyond The Surface Aesthetics is required by law to:
Maintain the privacy and security of your Protected Health Information (PHI)

We do not share mobile contact information with third parties or affiliates for marketing or promotional purposes. Information may be shared with subcontractors in support services, such as customer service. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
HOW WE MAY USE AND DISCLOSE YOUR HEALTH INFORMATION
For Treatment
We may use and share your PHI to provide, coordinate, or manage your medical and aesthetic care. This includes consultations, treatments, referrals, and communications among providers and staff.
For Payment
We may use and disclose PHI to bill for services rendered and to collect payment. This may include providing information to billing services, payment processors, or health plan representatives (if applicable).
For Healthcare Operations
We may use and disclose PHI for practice operations, including:

Staff training and credentialing

As Required by Law
We may disclose PHI when required to do so by federal, state, or local law, including public health reporting, audits, inspections, or law enforcement requests.
Business Associates

NEW JERSEY–SPECIFIC PRIVACY RIGHTS
In addition to HIPAA protections, New Jersey law provides enhanced privacy rights for patients. We comply with all applicable New Jersey statutes and regulations, including but not limited to:

New Jersey Confidentiality of Medical Information Laws
Minor Consent and Confidentiality Laws
Where New Jersey law provides greater privacy protections than HIPAA, we will follow the stricter standard.
ELECTRONIC COMMUNICATIONS
While we take reasonable steps to protect electronic communications, please note that:
Email, text messaging, and website forms may not be fully secure
By communicating with us electronically, you acknowledge and accept this risk
For sensitive or urgent medical concerns, we recommend contacting our office directly.
BREACH NOTIFICATION
In the event of a breach of unsecured PHI, we will notify you as required by HIPAA and New Jersey law, including providing details of the breach and steps taken to mitigate potential harm.
CHANGES TO THIS NOTICE
We reserve the right to change this Notice at any time. Any changes will apply to all PHI we maintain. The revised Notice will be available in our office and on our website.
