*Non-transferable. Includes a complimentary 40 units of your choice of, Xeomin, or Jeuveau every 90 days for a total of 4 free treatments per year. In the case that you miss your treatment for a specific quarter, you are allowed to roll-over those units to the next quarter. Any additional units exceeding the original 40 units will be $10/unit. Brand name Botox available at an additional cost. You can pre-purchase online or if you decide to sign up at the time of treatment, you will need to pre-pay the initial quarter at checkout.
